ADULT CARE

Hospital-level clinical care, delivered at home

Living with a complex health condition should not mean living in a hospital or care home. Our Adult Complex Care service provides nurse-led care in your own home, so you keep your routines, your relationships and your independence, with skilled clinical support around you every day.

Every package is assessed and led by a Registered Nurse, and delivered by a consistent team of healthcare assistants who have been trained and signed off as competent for your specific care. Unicare Complex Care Specialist Ltd is registered with the Care Quality Commission, including for the treatment of disease, disorder or injury.

WHO WE SUPPORT

Adults with long-term and clinically complex conditions

We support adults aged 18 and over whose health
needs require skilled, ongoing clinical care, whether
that follows a hospital discharge, a progressive
diagnosis, an injury, or a transition from children’s
services.

NURSE - LED AND HCA DELIVERED

Registered Nurses and healthcare assistants, working as one team

Our model pairs the clinical authority of a Registered Nurse with the continuity and value of a dedicated
healthcare assistant team. Nurses hold accountability for the care plan and for every clinical task
delegated; healthcare assistants deliver day-to-day care within the competencies they have been
signed off for. The result is clinical safety without hospital-level cost on every shift.

The Unicare approach

Person-centred care, with clinical governance behind it

We recognise that every adult we support is unique, so
every package is built around what matters to you —
not just your diagnosis. Our approach goes beyond
managing clinical needs to protecting quality of life,
independence and choice. We work closely with
everyone involved in your care, including:

HOW IT WORKS

From referral to a settled team at home

Enquiry and referral

Speak to our team or send an enquiry. We accept referrals from individuals, families, ICBs, local authorities, hospitals and case managers

Nursing assessment

A Registered Nurse assesses your clinical, personal and environmental needs at home or in hospital, and agrees the care plan with you.

Team matching and competency signoff

We build a team around you. Each healthcare assistant is trained and signed off by a nurse on your specific care before working alone.

Care begins, with ongoing nurse oversight

Regular nursing reviews, supervision and audits keep care safe, and the plan changes as your needs change.

QUALITY OVER SIGHT & CLINICAL GOVERNANCE

A named Clinical Care Lead for every package

Each adult we support is assigned a named Clinical Care Lead — a Registered Nurse — who is accountable for the quality of care delivered and remains your point of contact throughout your time with us.

We welcome your feedback. It helps us keep improving
the care we give.
