Unicare offers nurse-led care services that begin with a thorough assessment and the creation of a personalized care plan. The care plan is continuously monitored and adjusted as needed to meet the changing needs of the client. Nurses provide hands-on care alongside carers and maintain open communication with the family. They also offer guidance and support to promote safety and well-being. Collaboration with other care providers ensures a coordinated approach to achieving the client’s goals.

Tracheostomy Management

This includes (but is not restricted to) suctioning, tube tie and tube changes, stoma care, management of abnormal secretions, equipment and environment management, supervision and monitoring and humidification.

Why choose our Nurse Led care services?

Nurse Led care, or complex care as it is often known as, is a care service that is managed by a registered nurse. The registered nurse is responsible for overseeing any clinical care provided by the carer, ensuring that they are delivering the best service for the complex needs of the client. The level of clinical care offered to clients will depend on the type and complexity of the condition. Our carers and registered nurses are experienced in dealing with and providing care for a wide range of conditions including Parkinson’s and muscular dystrophy. We know that everyone is different, even if they are suffering from the same condition and as such, the nurse led care is always tailored to suit the individual.

What does nurse led care consist of?

With nurse led care you can expect monitoring of health and the recording of progress made with the condition. The nurse will work with other healthcare professionals to be able to provide a consistent, high quality care service, with a multi-disciplinary approach to help improve health outcomes for the client. The nurse will manage medications and associated administration, as well providing advice and support where required. The nurse will also provide advice on how to manage your conditions.

How do I get started with nurse led care?

You can contact us at any time to discuss your needs and we will create a tailored plan based on what you are looking for. We will always ensure that you receive a plan that is completely tailored to suit your needs

What are the clinical service options you provide?

We offer our clinical services to a wide range of people, including pediatric care for babies and children, as well as for complex conditions, such as Parkinson’s and cerebral palsy We can also provide palliative care for those that need it. Our nurse-led care includes feeding support and respiratory support.

How do I know what care I need?

You don’t need to worry about trying to work out what care services you need, we will do this for you. After you have contacted us, we will arrange a visit to find out more about your condition, and what we can do to help. We will create a tailored care plan based on your needs. It may be necessary to receive around the clock care, or you may need short term care. We are completely flexible in our approach to the nurse-led care we provide and are happy to adapt it to suit your needs.

How long does it take to set up the care plan?

We are in operation 24 hours a day, 365 days a year, so we are always on hand to deal with your queries, if you need care in an emergency care.

Will I still be able to see my friends and family?

Of course. In fact, we actively encourage this. Your family and friends can visit anytime they want, it is your home after all. Our carers can also take you to your family and friends if required, as well as any other activities you’d like to do.

How do you select a carer?

We provide carers that are best suited to your needs, taking into account your interests and the activities you enjoy, as well as your values. We believe this is paramount in ensuring you receive the right level of care, and above all, that it is enjoyable. Our carers often become friends with our clients, which is something we believe to be important to enable us to provide the best possible level of care.

Are the carers properly trained and experienced?

We only provide carers that are passionate about providing care and offering an exceptional level of service. Our carers are trained and experienced to do their job, and they must have experience to join us.
Our carers also receive clinical training to be able to provide any complex care at home. It allows them to be able to deal with clinical interventions and take a hands-on approach to Homecare. The training is delivered in a bespoke way, which allows us to ensure that the carers are trained specifically for the needs of each individual client.
We don’t just hire any carers, we only recruit those that are passionate about delivering care, and of course, that they are empathetic, caring people. It is our priority to ensure that the care we deliver is consistent and that it is always delivered to suit the needs of each individual.

What if I just want the care services as and when my condition is particularly bad?

Our care services are available 24/7 around the clock, in order to meet the needs of our clients. If you only need it during specific times or even just on an emergency basis we will be happy to accommodate your needs.
