We offer customised support for individuals with learning disabilities and/or autism who have additional needs, such as mental health conditions, challenging behaviors, or forensic backgrounds. Our commitment is to provide person-centered, flexible support that enhances choice and control. 

We collaborate closely with those we support and their networks to facilitate successful transitions from secure and inpatient settings back to their communities.

We also work with local partners to create and implement transition plans that address individual needs and promote positive risk management.

Step up support for people at risk of admission
  • Intensive Support: Delivered by highly skilled teams in safe and appropriate environments, ensuring preventative interventions are implemented.
  • Collaborative Approach: Working in partnership to facilitate the return to previous settings or to find alternative community solutions when individuals are ready.
Step-Down Solutions
  • Transitional Support: Assisting individuals as they move from inpatient settings into the community.
  • Independent Living Skills: Promoting the development of skills for independent living, facilitating community connections, and enabling access to local activities.
  • Housing Solutions: Sourcing and adapting housing to meet individual needs.
  • Health Action Planning: Supporting health and wellbeing outcomes in collaboration with local health services, guided by our commitment to the STOMP pledge.
  • Contingency Management Plans: Developing plans with individuals, their families, and professionals to prepare for and manage potential risks.
